General Summary

  • The Care Management Program Coordinator ensures the successful on-boarding of new providers and patients as they implement Chronic Care Management, Transitional Care Management, and evolving CMS Care Coordination or other programs for chronic care. Onboarding is the most important phase of the client life cycle, and the Care Management Program Coordinator is responsible for building and maintaining the client's satisfaction.

Essential Duties and Responsibilities

  • Identifies patients that meet the criteria for admission into the CCM/TCM program(s) or like programs.
  • Enrolls patients in TCM/CCM programs, developing a structured plan of care for CCM program members and setting a cadence for follow-up, monitoring, and education of all patients based upon medical needs, SOH, risk for readmission, and other unique needs.
  • Provides timely response to internal and external client inquiries via phone and email.
  • Maintains constant communication to and from the provider or designee in regards to patient emergent needs and changes in condition or plane of care. Implements interventions as directed by the provider.
  • Initiates Care Plans developed by care management program specialist / Licensed Practical Nurses.
  • Tracks program progress through enrollments, readmission rates, and other data.

Education

  • Graduate of an accredited school of nursing required.


Work Experience

  • Three years of experience in a healthcare setting.


Certification/License

  • Missouri or Compact RN license required. American Heart Association BLS required. Case management certification preferred.

Mental/Physical Requirements

  • Considerable mental concentration for sustained periods of time with frequent interruptions. Light lifting (15 lbs.) required. Standing, sitting and walking required.


Working Conditions

  • Potential exposure to body fluids and other events/substances present in a clinic/hospital environment. Typical medical clinic and hospital setting, with noise and distractions. Possible eye strain or other discomfort from constant use of computer screens.
