The Care Integration Senior Aide (CISA) will conduct visits to client homes to work directly with clients and with Direct Support Professionals on how to observe for, and help mitigate, worsening of client health or social conditions. The CISA will take information from these visits to client care teams to improve decisions about client care and support a reduction in preventable outcomes. The CISA will observe and maintain quality services throughout the Community Supported Living (CSL) program.

Job Responsibilities:

 1.       In-Home Visits

a.       Works with care managers to create, manage, and execute schedule for in-home visits

b.       Conducts in-home visits to:

                                                              i.      Observe and collect information regarding client’s health and social conditions

                                                            ii.      Mitigate worsening clinical or social conditions within scope of practice and in accordance with agency policy

                                                          iii.      Support home health aides in developing trusting relationships and establishing open communication with client and families using coaching techniques

c.       Records and reports observations from in-home visit and escalates concerns as needed

d.       Provides hand on direct support to members of the CSL program as necessary

 

2.       Care Team

a.       Actively participates as a member of the care team

b.       Reports observations from in-home visits and receives direction on follow-up activities from care team members

c.       Shares knowledge, perspective, and experiences as a caregiver with care team members

 

3.       Following-Up

a.       Collaborates on and completes follow-up activities delegated from care team members

b.       Continues to act as a support for direct support professionals and family caregivers

c.       Maintains member assessment plans to provide accurate and current information

d.       Trains direct support professionals within the scope of the care plan for each member served

e.       Provides updated direction to direct support staff as more current data is collected

4.       Professionalism

a.       Develops and maintains effective relationships with co-workers, clients and families

b.       Demonstrates ongoing commitment to comply with, support, and promote the agency’s policies and procedures

c.       Builds community-based relationships with stakeholders

d.       Interacts with and responds to staff in a professional and respectful manner

e.       Serves as a resource to direct support professionals

f.        Seeks to continually enhance knowledge of department-specific issues
