What You’ll Do:

• Deliver compassionate end-of-life nursing care in homes and facilities

• Assess patient needs, create care plans, and adapt as conditions change

• Support families with education, resources, and guidance

• Collaborate with physicians and the interdisciplinary team to ensure continuity of care

• Provide leadership by mentoring, supervising, and supporting team members

• Maintain accurate clinical documentation and uphold the highest standards of confidentiality and compliance

What We’re Looking For:

• Current RN license in Texas

• Bilingual (English + Spanish, Vietnamese, or Chinese)

• Valid driver’s license, auto insurance, CPR card, and ability to travel locally

• Passion for providing compassionate, patient-centered care

• Hospice or acute care experience preferred
