Lower end of range will apply to candidates who meet minimum experience or have limited years of relevant experience. The higher end of range will apply to seasoned candidates with considerable years of relevant experience.

PATH is seeking candidates passionate about helping others make a positive change in their lives to join our ICMS Department as a Re-Entry Case Manager, working out of our Los Angeles office on Madison Avenue.

ABOUT PATH
Since its foundation in 1984, PATH (People Assisting the Homeless) has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting our clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing. 

ABOUT OUR RE-ENTRY ICMS PROGRAM
PATH’s Re-Entry Intensive Case Management Services (RICMS) Program provides services to individuals re-entering society following criminal justice involvement. Re-entry intensive case management services include, but are not limited to, outreach/ engagement, case management, assessment and service planning, interim and permanent housing application, housing location, coordinating needed health services, and addressing all psychosocial service needs to stabilize clients in housing, reduce functional, health and mental health barriers, promote recovery, community integration and socialization. 

ABOUT THE CASE MANAGER
Case Mangers work as part of a multidisciplinary team comprised of other case managers, housing providers and subsidy funders, and community providers, including JCOD.  Within the RICMS Program, Case Managers are responsible for providing field-based, direct-client ICMS services designed to stabilize clients in their return to society.

Position Responsibilities include: 

  • Provide field-based and mobile case management services in the areas of, but not limited to: Independent living skills, housing stabilization, money management, community integration, employment linkage, benefits establishment, linkage to community providers for substance abuse, primary and mental health care, and all other services needed to assist clients in reaching their treatment plan goals.
