Job Description

AHRC NYC is looking to hire a Part-Time Physical Therapist for their Dickson-Goodman residence located at 126 East 126th St. in Manhattan. The pay rate for this position is $80-$85 per hour.

SCHEDULE: 8 hours per week....you pick the schedule!

The Physical Therapist is responsible for evaluating the person’s level of physical functioning and works with the person supported and residential staff to provide training and/or assistive devices to eliminate or reduce areas of deficit, usually associated with gross motor disorders and functioning.  The Physical Therapist also works with other professional staff to establish rehabilitation and maintenance goals and activities.

ESSENTIAL RESPONSIBILITIES

  • Evaluates physical therapy needs of people supported and provides physical therapy services as ordered by Physician/Nurse Practitioner.
  • Performs clinical tests to assess functional limitations and potential for restoration.
  • Develop a plan of care with restorative and/or maintenance goals to meet identified needs.
  • Performs applied physical therapy procedures to achieve identified goals (e.g. therapeutic exercises, functional training).
  • Teaches the use of assistive modalities and appropriate physical therapy procedures to the person supported and residential staff to achieve maximum effect.
  • Conducts ongoing communication with the Physician/Nurse Practitioner concerning the person’s reaction to treatment and any changes in the person’s condition.
  • Ensures initial evaluations are completed upon admission.
  • Makes physician contact, followed by written reports, documenting initial findings, goals, treatment frequency, and estimated length of service.
  • Completes written progress reports to be filed in a person’s record.
  • Fits assistive devices for self-care and ambulation, trains person supported and staff in use of devices, and monitors for continued need and ability to use.
  • Participates in interdisciplinary care planning for individual needs.
  • Consult with other members of the health team to coordinate therapeutic programs for the people supported in assigned caseloads.
  • Completes all necessary documentation within seven (7) days of service.  Documentation includes the type of treatment given, the modality used, and the person’s reaction.
  • Submits discharge summaries at completion of services, including care provided, equipment ordered, and recommendations for follow-up care.
  • Participates in Quality Assurance activities, as requested.
  • Conducts other related tasks as needed or required.

Qualifications

  • Current licensure in New York State as a Physical Therapist.
  • Graduate of Program in Physical Therapy Curriculum, approved by the American Physical Therapy Association or Council of Medical Education of American Medical Association.
  • Minimum of one year of Physical Therapy experience in a health care setting.
  • Demonstration of considerable knowledge of current standards, procedures, and techniques of Physical Therapy.
  • Strong interpersonal, written, and verbal communication skills.
  • Proven capacity to work productively in highly collaborative settings and to seek and synthesize input from multiple stakeholders.
