Job Overview:
Responsible for the delivery of patient care through the nursing process of assessment, diagnosing, planning, implementation, and evaluation. Responsible for directing and coordinating all nursing care for patients in ambulatory setting based on established clinical nursing practice standards. Collaborates with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes. Supports the organization vision and mission. Utilizes knowledge of patient’s age and cultural diversity into the provision of patient care. Contributes to the provision of quality nursing care through performance improvement techniques that demonstrate positive outcomes in patient care.
Job Requirements:
Required:
• Graduate of an accredited school of professional nursing.
• Licensure in the State of Ohio as a Registered Professional Nurse (RN).
Preferred:
• A bachelor’s degree in nursing is preferred.
• PICC line insertion certification preferred. Otherwise PICC Line insertion training/competency must be accomplished within 3 months of employment.
AGE/PATIENT SPECIFIC COMPETENCIES:
• Toddler – 1-3 Years of Age
• Preschool – 3-5 Years of Age
• School Age – 6-12 Years of Age
• Adolescent – 3-18 Years of Age
• Adult – 19-65 Years of Age
• Elderly – Over 65 Years of Age
Job Responsibilities:
ESSENTIAL DUTIES & RESPONSIBILITIES:

In collaboration with the physicians and APN and in accordance with hospital policies, procedures and standards of care will:
Obtain health history
Perform nursing assessments
Request, perform and interpret laboratory studies
Implement appropriate plan of care
Assess effectiveness of plan of care
Provide appropriate patient education and discharge planning
Maintain a safe environment for patients and personnel by utilizing and enforcing principles of safety and infection control, including universal precautions.
Transcribe and execute orders
Schedule cases
Administer IV, hypodermic and oral medications. Monitor and document response.
Initiate and monitor administration of IV fluids and blood products.
Document all care utilizing the nursing process and compliance with hospital policies and procedures.
Report emergent or unusual patient response or condition immediately to manager and other members of the team as appropriate.
Maintain open communication with manager, assuring he/she is informed of activities and all concerns.
Assist with patient assessment and selection, which includes determining the patient clinical stability, acceptance of the treatment plan and requirements, home environment and support, home-bound status, psychosocial issues and reimbursement plan.
Assist in patient enrollment and education. This includes obtaining and documenting informed consent, notifying the patient of financial responsibility for co-pays, educating the patient about the treatment plan.
Assist in writing policies and procedures related to the program.
