Required Qualifications (as evidenced by an attached resume):
Bachelor’s degree (foreign equivalent or higher) in Nursing or related medical field. Current New York State Registered Nurse. Three (3) years of full-time Registered Nurse experience in a clinical, hospital or ambulatory setting. Administrative experience in a healthcare setting.
Preferred Qualifications:
Master’s Degree (foreign equivalent or higher) in Nursing or Medical field. Case management experience. Experience auditing medical records and providing data reports to management. Experience with the following patient populations: PTSD/depression, cancer, lung disease, sleep apnea and/or cognitive decline. Familiar with the WTC Healthcare Program and its processes.
Brief Description of Duties:
The RN Administrator is a clinical leader who delivers high-quality, patient-centered care while overseeing care coordination for individuals with complex and chronic conditions. This role blends direct patient care with program leadership, requiring the ability to pivot based on clinical and operational needs. The RN Administrator mentors and supervises clinical staff, including LPNs and Medical Assistants, ensuring adherence to standards of care and regulatory compliance. Serving as a central point of coordination, the RN Administrator manages patient care across the continuum - conducting assessments, facilitating timely interventions, and addressing barriers to care. They lead quality improvement initiatives focused on enhancing access, closing care gaps, and improving patient outcomes, while collaborating with interdisciplinary teams to drive program success.
Duties:
- Deliver comprehensive patient care and care coordination for individuals with complex and chronic health conditions, ensuring continuity and quality across clinical services.
- Educate patients on diagnoses, treatment plans, cancer screenings, and program benefits to promote adherence and informed decision-making.
- Perform clinical duties including phlebotomy, spirometry, injections, vital signs, and medical questionnaires and cognitive assessments.
- Serve as a primary point of contact for patient inquiries via phone and patient portal; triage clinical concerns and coordinate timely follow-up care.
- Conduct patient assessments to evaluate acuity, barriers to care, and eligibility for case management services.
- Manage and monitor patient cases, including hospitalizations, discharge planning, referrals, and pre-authorizations for tests and treatments.
- Collaborate with interdisciplinary teams - including providers, social workers, pharmacists, and case managers - to ensure coordinated, patient-centered care.
- Support patient adherence by identifying care gaps, conducting outreach, and facilitating appointments with internal and external providers.
- Quality control and research:
- Manage and track quality measures, including cancer screenings (e.g., Cologuard, lung cancer screening, mammograms, colonoscopies), ensuring timely follow-up and documentation of results.
- Assist with DME audit logs and collaborate with the Care Coordination team to ensure patients receive necessary durable medical equipment.
- Maintain accurate and detailed documentation in the EMR, including patient histories, clinical data, and care plans.
- Participate in quality assurance and performance improvement initiatives, including chart reviews, audits, data collection, and compliance monitoring.
- Assist with research data collection, certification processes, and program reporting requirements.
- Participate in committees to support clinic engagement, collaboration, and continuous improvement initiatives.
- Perform other duties as assigned.
