About the role

Essential Functions - Psychiatric Mental Health Nurse Practitioner (PMHNP)

  • Provides outpatient psychiatric services for all patients registered to Spectrum Health Services, Inc.
  • Provide for the non-obstetrical needs of the site’s adult and pediatric population
  • Examines patients for symptoms or physical information
  • Follow up on abnormal laboratory results
  • Reviews patient’s medical record, including allergies, problems, medications, and immunization status
  • Elicits and records information about patient’s medical history
  • Conducts psychiatric assessments to provide information needed for health wellness, admission to school, consideration for jobs, or eligibility for insurance coverage.
  • Documents the patients visit including physical and psychiatric exam, diagnoses, and plan of action.
  • Promotes health by advising patients about diet, hygiene, and methods for preventing of disease.
  • Refers patients to medical specialist for consultant services when necessary for patients well being
  • Confers with staff of other agencies, hospitals, schools and community services to establish and maintain continuity of care.
  • Responds to and assists with any medical emergencies that may occur at the site.
  • Assists in establishing of psychiatric clinical protocols for Spectrum Health Services, Inc
  • Participates in Quality Improvement activities and peer review activities as required.
  • Must be able to work collaboratively in an interdisciplinary based team model with other clinicians
  • Perform diagnostic assessments, psychopharmacologic evaluations, coordination of care with multidisciplinary providers and relevant clinical documentation
  • Engage with the health center’s Psychiatrist for monthly supervision and as needed
  • Provide prescriber coverage to primary care patients within the health center practice
  • Collaborate with all Clinical Providers and program leaders to support the efficient assessment and care of behavioral health patients
  • Maintain all appropriate licensure to prescribe and dispense medications under applicable state and federal laws 
  • Have excellent grammar, attention to detail, efficient at writing medical information at a patient level 
  • Must be technology savvy and excited about how technology can improve medical care 
  • Provide direct patient care via telemedicine (remote) 
  • Evaluate patient records, make prescribing decisions, and provide medication-related counseling as needed in a timely manner 
  • Support a 72-hour messaging response window to patients 
  • Works in collaboration with the collaborating physician(s) in accordance with state law, health center policy, protocols and clinical pathways and may provide autonomous team-based care.
  • Educates patients/families on care and recommended or available resources. In collaboration with the supervising/collaborating MD, this may be inclusive of admitting instructions, treatment plan, actual and potential side effects of treatments/medications, and discharge plan.
  • Provides psychosocial support to patients, their family members, and significant others in a caring, ethical, and professional manner.
  • Ensures appropriate documentation within the accordance of health center policy and regulatory requirements and protects information of all forms, whether computer-based, paper, film, voice, or other media from unauthorized access modification, destruction, or intentional or accidental disclosure.
  • Demonstrate a high standard of moral and ethical behavior, professionalism, compassion, and commitment to patient care.
  • Ensures compliance with patient related healthcare regulatory requirements, standards and recommendations.
  • Engages in projects that maintain and advance professional competency.
  • Serves as a clinical resource for Medical Providers and may provide service specific orientation pertinent to clinical education of new Providers
  • Consistently demonstrates high reliability organizational behaviors through reporting safety events as well as near misses.
  • Other duties as assigned
