Adventist HealthCare's Post-Acute Care Center seeks to hire an experienced Business Development Clinical Liaison for our Post-Acute Care Services Department who will embrace our mission to extend God’s care through the ministry of physical, mental and spiritual healing.

As a Business Development Clinical Liaison you will:
• Establishes and fosters strong collaboration with ongoing and potential referral sources; partners with external case managers to provide seamless transition to Home Health.
• Designs and develops business development strategies for his/her assigned territory designed to increase patient admissions that leverage AHC Home Health core competence in the area of rehabilitating medically complex patients.
• Promotes effective messaging regarding Home Health regionally recognized programs and services to patients, families, healthcare providers and the community
• Develops and executes presentations and in-services alongside Business Development Manager and Director. Utilizes appropriate materials and messaging to promote programs and services to patients, families, and referral sources
• Collaborates with the Inpatient Documentation Liaison to provide coordination of a smooth transition from the referring facility to the Home Health setting.
• Understands rehabilitation diagnoses and clinical indicators, coordinating with Documentation Liaison, in order to make appropriate decisions for admission
• Expedites flow of admissions through collaboration with the Patient Access Clinical Coordinator, physicians, pharmacy and nursing
• Facilitates ongoing communication with patient/family to prepare for admission

Qualifications include:
• RN, PT, OT, SLP degree required
• BSN degree preferred
• 3-5 years clinical experience in hospital setting. Preferred 3-5 years as a Clinical Liaison or case manager
• CCRN or CCM preferred, Active Maryland license as Registered Nurse (RN) or • Licensed Practical Nurse (LPN), Physical Therapist (PT), Occupational
• Therapist (OT), and Speech Language Therapist (SLP).
• Valid driver’s license and ability to travel between hospitals

Work Schedule:

Monday-Friday 8am -5pm with holidays and weekends covered as needed.

Pay Range:

$71,932.12 - $107,889.60

In cases where the range is displayed as a $0 amount or if the position is Per Diem (offering a fixed rate), salary discussions will occur during the screening process.
