Upperline Health providers coordinate patients’ care among a team of specialists – physicians, advanced practice providers, care navigators, pharmacists, dieticians, and social workers for integrated treatment that addresses patients’ immediate and long-term health needs. Upperline Health is based out of Nashville, TN and has operations in 130+ clinic locations across Alabama, California, Florida, Georgia, Indiana, Kentucky, and Tennessee.
We put patients at the center of value-based care.
This position requires travel up to 50% of the time to partner practices around the East Coast to Mid-West. This position is available to remote applicants who reside in the following states: AL, FL, GA, IL, IN, KY, MI, SC, NV, PA, TN, TX.
About the Practice Success Partner
Upperline Health is seeking a Practice Success Partner to guide partner practices through the Value Based Care program enrollment, ensuring understanding of processes, documentation, and technology.
What You'll Do:
- Onboarding: Guide partner practices through the Value Based Care program enrollment process, ensuring understanding of enrollment process, required documentation, applicable technology (hardware and apps), forms and compliant processes. Conduct onboarding sessions with clinic leads and doctors to educate partners about program expectations, available resources and guide practices through resources to ensure successful accomplishment of key milestones.
- Data Tracking and Performance Mitigation: Analyze enrollment data to identify trends and opportunities for improvement. Partner with partner clinics to understand root cause of issues mitigation plans (including additional training). Communicate performance metrics to all partner clinics.
- Partner Engagement: Build strong relationships with partner clinics to foster trust and engagement in the program. Provide ongoing support and education to patients throughout their participation.
- Collaboration: Collaborate with care teams, providers, and other stakeholders to ensure program success.
- Quality Improvement: Contribute to process improvement initiatives to enhance partner enrollment efficiency and effectiveness. Communicate improvement opportunities to internal leadership.
Required Skills and Competencies:
- High school diploma or equivalent; associate's or bachelor’s degree preferred.
- Previous experience in healthcare is required; Knowledge of value-based care programs is preferred.
- Previous experience deploying programs with a technology component
- Excellent communication and interpersonal skills, with the ability to build rapport quickly across all areas of clinic staff engage through story-telling
- Demonstrated ability to provide training in an effective manner
- Demonstrated ability navigating conflict
- Strong organizational and time management skills.
- Proficiency in computer systems and data entry.
- Ability to work independently and as part of a team.

Compensation
Compensation is commensurate to compensation for similar positions in the region and based on prior training and experience
