About the role

The RN Care Manager provides care coordination services for hospital patients to facilitate optimal patient outcomes, supports the delivery of effective and efficient care, improve continuity of care and efficient use of resources throughout the episode of care. The care manager will collaborate with the care team including, but not limited to attending/specialty physician(s), social worker, utilization management, therapies, and nursing to ensure that an effective and timely treatment plan is acted upon, and an appropriate discharge plan is implemented with a smooth transition in levels of care. Provides care management services on periodic weekends and/or holiday(s) as assigned.

Required Qualifications:

  • Associate’s degree in nursing OR Bachelor’s degree in Social Work. 
  • Current RN licensure from the State of Minnesota Nursing Board OR Licensed with Minnesota Board of Social Work with minimum license level of Licensed Social Worker (LSW). 
  • Excellent time management skills
  • Basic computer skills
  • Function independently and as part of a team, working effectively with various facilities, internal and external staff, patients, patients’ family, and physicians to facilitate patient care
  • Excellent verbal, written and interpersonal skills
  • Excellent problem identification, problem-solving skills, and follow through skills
  • Ability to deal with change and ambiguous situations
  • Able to work with individuals of diverse backgrounds

Preferred Qualifications:

  • If an RN: Bachelor’s or Master’s degree in Nursing 
  • Three (3) years’ experience performing case management and/or discharge planning. 
  • One-year prior experience working as a registered nurse in public health, home care, skilled nursing facility or acute care setting. 
  • Experience with utilization review criteria and/or progression of care guidelines 
  • Knowledge of electronic health records
