JOB DESCRIPTION:
The PHSO Program Coordinator supports the University of Vermont Health Network Population Health Service Organization (PHSO) in providing strategic, operational support to providers and care teams throughout the network to support PHSO and other HVC initiatives and priorities. The incumbent will assist with ongoing PHSO Program Coordination, creating high-value process improvement and quality projects by using data to identify opportunities for system-level change to impact gaps in care. This role supports efforts to improve the delivery of care to patients and to enhance the organization’s overall performance in the HVC and Value-based Contracts.
EDUCATION:
Bachelor's degree in a healthcare-related field (e.g. nursing, social work, health care administration, public health) required. Master’s Degree preferred.
EXPERIENCE:
Minimum of five years’ experience in a related field. Demonstrated experience in project management, stakeholder engagement, interpreting and evaluating clinical data and communicating findings to varied audiences.
