Home Health Physical Therapist (PRN)

Responsibilities

• Perform evaluations and assessments to develop individualized plans of care for patients in their homes.
• Develop and implement comprehensive treatment plans that include exercise, therapeutic activities, manual therapy, and other modalities.
• Monitor and document patients' progress toward their goals and adjust treatment plans accordingly.
• Educate patients and their families on the importance of therapy and home exercise programs, as well as safety and injury prevention techniques.
• Collaborate with interdisciplinary teams, including physicians, nurses, and other therapists, both within the organization and in the community.
• Maintain accurate and timely documentation.

Requirements

• Current state license in Michigan to practice physical therapy.
• Minimum of 1 year of experience as a PT in home health or related setting preferred, but not required.
• Ability to work independently and manage time effectively.
• Strong critical thinking, problem-solving, and communication skills.
• High level of professionalism and attention to detail.
• Ability to use electronic medical records and other technology to document and manage patient care.
• Reliable transportation and auto insurance, as this position requires travel.
