• Ensures that all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Home Health team. • Conducts start of care and other visits with all assigned clients to verify that client’s service plan is being followed (in accordance with State/Federal requirements). • Monitors and reports significant changes in client’s mental and physical status to the client’s assigned RN Manager, case worker, or the client’s physician in cases of emergencies. • Supervises attendant’s work to ensure that client’s service plan requirements are being met. • Refers clients for skilled care services when necessary. • Evaluates each client’s surroundings for fire and other safety hazards, develops and recommends plans to abate such hazards; coordinates such plans with contract case workers and the caregiver as appropriate. • Conducts attendant performance assessments during client visits as needed. • Schedules appropriate services for all requests for homemaking, personal care, and home health aide services in a timely manner in accordance with established procedures, including staffing planned and unplanned absences. • Maintains accurate records of the current and prospective requested services, tasks and/or plan of care1 for all clients. • Maintains accurate records of the current and prospective schedule for all caregivers Reviews and maintains contract specific documentation for applicable clients. • Informs and educates each client on pertinent information required by Elara Caring policy and Federal/State regulation. • Actively participates in and cooperates with strategic growth initiatives for the branch. • Reviews and utilizes reports to identify and correct errors on a timely basis to ensure correct billing and payroll. • Processes and manages employee time slips to ensure accuracy (as needed). • Implements Elara Caring policies and procedures. • Attends training sessions as directed. • Available to provide qualified direct care services as needed. • Maintains compliance with State, Federal, licensure and contract related regulatory requirements. 1 For Texas, references to plan of care are considered individualized service plans for regulatory purposes. • Provides support, assistance and direction to caregivers as needed. • Ensures in-services, trainings, health assessments, medical requirements, certification requirements for caregivers are compliant with State, Federal, and Elara Caring requirements. • Works with and counsel’s caregivers to improve and correct performance. • Communicates verbal and written disciplinary action for caregivers (with the assistance of HR as needed). • Notifies caregivers of problems or new developments in assigned area. • Assists Talent Acquisition team with recruiting caregivers, including community events as needed. • Maintains documentation and records for each caregiver and client. • Conducts caregiver orientations on supervised clients (Texas, Oklahoma, Missouri, Massachusetts, Michigan, Illinois, Indiana, Louisiana only). • Cooperates with timekeeping and payroll departments in the collection of outstanding attendant timesheets. • Monitors client service delivery requirements daily to ensure adequate availability of attendants. • Monitors systems throughout the day to ensure authorizations match scheduled services. • Reviews over- and under-utilization reports and addresses inconsistencies on a daily basis. • Reviews and submits employee allegation and worker’s compensation injury information to HR (or appropriate authority) in a timely manner. • Reviews open shifts daily to ensure shifts are staffed appropriately for care delivery. • Communicates with employees, case managers and clients in a professional and effective manner. • Consults with Manager through written and verbal reports concerning any employee or case manager issues. • Follows established company policies and procedures, while representing the company in a professional and positive manner
