Your Specific Responsibilities:

This position works within the Community Health Improvement Section of the Okaloosa County Health Department (OCHD). Work includes promoting, maintaining, and improving individual and/or community health by assisting individuals and communities to adopt healthy behaviors. Complex tasks are performed under limited supervision where general policies may not have been previously established and standard methods may not be available. The job requires an extensive knowledge of community dynamics and an ability to be an effective agent for change. This position will serve as a resource to assist individuals, other professionals, and the community to make policy and systems changes associated with Chronic Disease prevention.

 

Promotes the Community Health Improvement initiatives among community partners, businesses, childcares, faith-based organizations and individuals. Provides direct support to community partners to ensure successful implementation of health promotion strategies. Communicates extensively with persons outside the organization. Tailors communication to the needs of the audience. Represents the organization to the public, government, and other external sources. Provides information to supervisors and fellow workers. Information can be exchanged face-to-face, in writing, or via telephone/electronic transfer. Serves as primary for plan development in Healthy Faith-Based, and Worksites in addition to various Community Health Improvement Plan (CHIP) groups within the Okaloosa County Health Improvement Partnership (OCHIP). Serves as backup for Healthy Okaloosa Child Care and various CHIP groups within the OCHIP. Plans include strategies to reduce health disparities and address the social determinants of health. Completes CHIP group documentation, including action plans and meeting minutes.

 

Collaborates with health specialists and civic groups to ascertain community health needs, determine availability of services, and to develop goals using a written work plan. Promotes health discussions in schools, industry, community agencies and faith-based organizations to encourage policy or systems changes related to chronic disease prevention though the Healthiest Weight initiative or other Community Health Improvement Plan (CHIP) goals, strategies and objectives. Mobilizes the community to define and address prevention and systems change and develops partnerships to address CHIP goals.

 

Conducts activities to increase awareness of the impact of lifestyle on healthy weight, chronic disease and other community health initiatives. Develops and maintains cooperation between public, civic, professional, and voluntary agencies. Prepares and disseminates educational and informational materials that are linguistically and culturally appropriate. Develops constructive and cooperative working relationships with others. Provides consultation, education and expert advice on Healthiest Weight program components and other community health initiatives, including technical, systems-related, or process related topics. Translates or explains information in a way that members of the community can more easily understand issues related to Healthiest Weight, chronic disease prevention and other community health initiatives. Assists community members to establish systems for implementing community health initiatives, such as 5210. Implements plans for Healthy Okaloosa sectors, community health improvement plan groups. 

Collects and analyzes data to identify individual and/or community needs prior to planning, implementing, monitoring, and evaluating programs or services designed to encourage healthy lifestyles. Conducts community surveys to ascertain health needs, develop desirable health goals, and determine availability of resources. Uses information and data to recognize differences or similarities and sense changes in circumstances or events. Modifies behavior or approach based on this information. Develops multiple approaches for implementing plans in a culturally competent and appropriate manner.
