The Care Manager, Telephonic Nurse 2 , in a telephonic environment, assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Manager, Telephonic Nurse 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.The Care Manager, Telephonic Nurse 2 employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues.
Identifies and resolves barriers that hinder effective care
Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations.
May create member care plans
Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas
Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed
Follows established guidelines/procedures

Required Qualifications
Unrestricted Current Licensed Registered Nurse (RN) in the state of South Carolina or a compact license with no active disciplinary action
3+ years of clinical acute care experience
1 year Case Management, care coach, or care coordination
Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook
Preferred Qualifications
Bachelor's degree
Experience with case management, discharge planning and patient education for adult acute care
Managed care experience
Behavioral Health Experience
Certified Case Manager (CCM)
Live in the state of SC

Workstyle: Remote Work at Home
Location: South Carolina Preferred
Work Schedule: Monday - Friday; 8:00 AM - 5:00 PM Eastern Time
Travel: 5%, occasional travel up to 1 time per quarter

· Associates who live and work from Home in the state of California, Illinois, Montana, or South Dakota will be provided a bi-weekly payment for their internet expense.
· Humana will provide Home or Hybrid Home/Office associates with telephone equipment appropriate to meet the business requirements for their position/job
· Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information
