The Clinical Pharmacy Coordinator – Informatics and Medication Safety is responsible for updates to and maintenance of pharmacy systems and databases, collaboration with medical, nursing and Information Technology staff on issues regarding the pharmacy Physician Order Management (CPOE), EMAR, Bedside Medication Verification, IV Compounding system, Automated Dispensing Machines, Inventory Management system, and infusion pumps systems. The individual is responsible for coordinating medication safety initiatives within the pharmacy department, while collaborating with the Department of Quality and Safety. They will co-chair the hospital’s Medication Reliability Committee and work collaboratively with Director of Quality and Patient Safety to review and analyze safety events. Additionally, the individual will serve as the Mount Auburn Hospital liaison on the Beth Israel Lahey Health System Medication Safety Committee.

Job Description:

Essential Duties & Responsibilities (including but not limited to):

1. Develops and maintains clinical programs in conjunction with Nursing and medical staff to optimize the pharmacotherapeutic aspects of patient care.

2. Experience with Epic Pharmacy Module (Willow), Pyxis, and Logistics. Epic Willow Certification not required

3. Able to utilize personal computer applications such as Microsoft Word and Excel

4. Demonstrates functional job knowledge in the preparation and dispensing of medications and intravenous admixtures in the hospital normally acquired in one year of hospital Pharmacy experience, which may be acquired as an intern prior to graduation

5. Verbal, quantitative, and interpersonal skills to communicate effectively with members of the allied health disciplines

6. Works accurately under stress and meets deadlines

7. Must have good oral and interpersonal communication skills, to communicate effectively with members of other healthcare disciplines, and other Pharmacy staff

8. Must be able to plan and coordinate the activities pharmacy staff in accordance with Policy and Procedures

9. Priorities and organizes work while under stress and with time constraints. Must pay attention to detail virtually 100% of work time, and have critical thinking and decision making skills

10. Must be able to set and change priorities, and accept additional responsibility as needed

11. Must have extensive knowledge and ability in all areas of the department including: IV Admixtures, Drug Information, Ambulatory, Manufacturing, and Drug Distribution

12. Must be able to train new employees

13. Must be capable to assume the responsibility for coordinating pharmacy services on an assigned shift

14. The individual in this position must demonstrate appropriate knowledge of principles of growth and development over the life span of the neonate, adolescent, adult, and geriatric patient. In addition,

they must possess the ability to assess patient data relative to age specific needs and provide care as described in the department's policies/procedures

15. Must have comprehensive knowledge of Pharmacotherapy

16. Ability to work collaboratively as a member of the patient care team.

Minimum Qualifications:

1. Required to be able to perform order entry and utilize all Registered Pharmacist functions of the Pharmacy computer system accurately and efficiently. Bachelor of Science in Pharmacy (five-year degree program) from an accredited college of pharmacy and licensure to practice in the Commonwealth of Massachusetts from the Board of Registration in Pharmacy

2. Graduate of accredited school of Pharmacy Licensed to practice Pharmacy in Massachusetts.

3. At least 3 to 5 years hospital pharmacy experience required

4. Pharmacy residency or comparable experience preferred

5. Board Certified Pharmacotherapy Specialist or comparable certification preferred

6. ASHP Medication Safety Certificate or comparable certification preferred

7. Requires independent decision-making, clinical judgment, and in-depth knowledge of oncology and therapeutics.

Physical Requirements & Environment:

1. Normal hospital environment with possible rotating shifts

2. Works under constant time deadlines

3. Occasional exposure to infectious disease and to possible carcinogenic, mutagenic, and sensitizing medications. Minimal exposure to caustic agents, acids, and fumes.
