About the role

In this role, the Advanced Heart Failure Outreach Coordinator will focus on increasing referrals into the TCH Network for evaluation of advanced HF therapies.  The individual will coordinate meetings with physicians in the community to develop relationships with our physicians. The coordinator will help streamline the process of referral and evaluation such that patients and referring providers are satisfied with the timeliness and communication from the team.  The Program Coordinator will also assist in the planning and execution of educational and community outreach events to increase awareness of heart transplant, LVADs and total artificial hearts in our tri-state region. 

Promote referrals for Heart Transplant and LVAD

  • Develop and implement plan to collaborate with community physicians to help drive referrals of their patients with advanced heart failure into our center.
  • Coordinate and facilitate meetings and events with outside physicians with the TCH advanced HF surgeons and cardiologists.

Community Education & Outreach

  • Develop and implement education for advanced therapy teams and provide to the community.
  • Participate in organ donation promotion campaigns in our community.
  • Develop & provide first responder education on advanced therapies.
  • Collaborate with marketing team and advanced therapies teams in the development of educational materials and social media campaigns.
  • Collaborate with IT and marketing to enhance the website promoting Heart Transplant and LVAD programs.
  • Partner with community leaders to identify venues and events where education on advanced therapies can be included, i.e. health fairs, conferences, or gatherings

Program Data Analysis

  • Develop methodology and implement tracking the advanced HF program volumes and outcomes in collaboration with advanced therapy teams.
  • Analyze data and outcomes to present to all stakeholders.
  • Develop and implement market strategy identifying focus areas and markets

Administrative Duties

  • Create presentations of data for teams.
  • Arrange meetings in person and virtual as needed.
  • Responsible for emails, mailings, phone calls, and follow-up
  • Assist with internal planning of educational programs (Grand rounds, symposiums, CME)

EDUCATION:    BSN required. Master’s degree preferred.                          

YEARS OF EXPERIENCE: Prior program coordinator in cardiology and/or healthcare program operations experience required. 2-3 years working as part of an advanced interdisciplinary team (examples: CVICU, LVAD, Transplant, MCS).

Strong communication, problem solving, and critical thinking skills

Capable of collecting and analyzing data, recognizing opportunities for improvement, and formulating / implementing plans for change

Ability to collaborate and develop positive relationships with physicians and coworkers

Ability to adapt various technologies and a rapidly changing environment

Proficiency in Microsoft applications (i.e., Word, Excel, Outlook, and PowerPoint)
