Job Summary:
Charter Oak Health Center is seeking a highly skilled Women's Health Nurse Practitioner / APRN to join our team. As a Nurse Practitioner / APRN you will be responsible for providing clinical services to clients, including professional diagnosis, prescribing and administering treatment, and offering treatment options as part of a multi-disciplinary team of case managers, therapists, nurses, and program managers.
Key Responsibilities:
- Work independently and as part of a team, collaborating with providers to conduct age-appropriate histories and physical exams for complex, acute, critical, and chronically ill patients.
- Order and interpret diagnostic and therapeutic tests based on age-specific needs.
- Prescribe pharmacologic and non-pharmacologic treatments.
- Implement interventions to help patients regain or maintain stability.
- Monitor the effectiveness of interventions through follow-up.
- Perform various procedures (based on competency), including injections, minor surgeries, PAP smears, cultures, EKG tracings, phlebotomy, and eye irrigation for foreign bodies.
- Collaborate with multidisciplinary team members for appropriate referrals.
- Provide educational tools to staff, patients, and families to aid in decision-making.
- Perform other duties as required by management.
Requirements
Qualifications:
- Master's degree in a Nursing and current licensure in the state of CT.
- Minimum of 2 years of experience in ambulatory care setting preferred
- Board Certification as an APRN; current Nurse Practitioner certification in Women's Health
- Current provider card in Basic Life Support; CPR certified
