Provides physical therapy treatment to patients who have impairments, functional limitations, disabilities, or changes in physical function and health status resulting from injury, disease or other causes under the direction of a Physical Therapist. Actively collaborates with the health care/educational team to meet patient care, outcomes and system improvement objectives.

Schedule:

Typically Monday–Friday, 8:00 AM – 4:30 PM
Flexible 4- or 5-day workweek options available

About the Role

Vanderbilt Health is seeking a compassionate and skilled Physical Therapist Assistant (PTA) to join our Home Health team. In this role, you will provide high-quality, evidence-based physical therapy treatments to patients recovering from injuries, illnesses, or chronic conditions. You’ll work under the supervision of a licensed Physical Therapist and collaborate with a multidisciplinary team to support patient recovery and independence in the home setting.

Key Responsibilities

• Deliver individualized physical therapy interventions based on the supervising PT’s treatment plan

• Promote patient safety and functional independence in the home and community

• Educate patients and caregivers on therapeutic exercises, mobility, and home safety

• Accurately document patient progress in compliance with VUMC and regulatory standards

• Collaborate with nurses, physicians, and allied health professionals for coordinated care

• Travel to patient homes in Rutherford, Sumner, and East Davidson Counties

Qualifications

Required:

• Active Physical Therapist Assistant (PTA) license in Tennessee

• Minimum 1 year of experience as a PTA

• Valid driver’s license and reliable transportation

Preferred:

• Proficiency with electronic medical records (EMR)

• Strong understanding of rehabilitation techniques and functional mobility

• Excellent communication and interpersonal skills

• Ability to work independently and manage a flexible caseload
