💼 Position Overview

As a Home Care Nurse at Affirmed, you'll provide direct care to patients in their homes — delivering the same high-quality nursing you'd expect in a clinical setting, but in a much more personal environment.

Responsibilities include:

• Providing individualized nursing care based on each patient’s care plan

• Administering medications, monitoring vitals, and managing complex needs as appropriate (e.g., wound care, catheters, colostomies)

• Maintaining accurate and timely documentation

• Offering emotional support to clients and families

• Collaborating with the Director of Nursing to ensure high standards of care

📌 Qualifications

• Valid RN or LPN license in the state of [Insert State]
