Physical Therapist Assistant (PTA) - Home Health

• Service Areas: Rutherford, Sumner, and East Davidson Counties

• Schedule: Usually Mon–Fri, 8:00 am – 4:30 pm, but 4 & 5-day a week options are available

Vanderbilt Health is seeking a compassionate and skilled Physical Therapy Assistant (PTA) to join our Home Health team. In this role, you will provide high-quality, evidence-based physical therapy treatments to patients with physical impairments, functional limitations, or disabilities resulting from injury, illness, or chronic conditions. You will work under the supervision of a licensed Physical Therapist and collaborate with a multidisciplinary healthcare team to support patient recovery and independence in the home setting.

How You’ll Make a Difference:

• Deliver individualized physical therapy interventions based on the treatment plan developed by the supervising Physical Therapist.

• Promote patient safety and functional independence in the home, community, and vocational environments.

• Educate patients and caregivers on therapeutic exercises, mobility strategies, and home safety.

• Accurately document patient progress and treatment outcomes in compliance with VUMC and regulatory standards.

• Collaborate with nurses, physicians, and other allied health professionals to ensure coordinated care.

• Travel to patient homes within Rutherford, Sumner, and East Davidson Counties.

Required Qualifications

• Active Physical Therapist Assistant (PTA) license in the state of Tennessee.

• Minimum 1 year of experience as a Physical Therapist Assistant (PTA)

• Valid driver’s license and reliable transportation.

Preferred Skills & Competencies

• Proficiency in electronic medical records (EMR) and documentation systems.

• Strong understanding of rehabilitation techniques, functional mobility, and evidence-based therapy.

• Excellent communication and interpersonal skills.

• Ability to work independently and manage a flexible caseload.

KEY RESPONSIBILITIES
• Provide treatment as specified on the written treatment plan by the supervising Physical Therapist. Demonstrates ability to:
• Administer various therapeutic modalities according to departmental protocols and guidelines. These include ultrasound, whirlpool, T.E.N.S., biofeedback, hot/cold packs, paraffin baths, electric stimulation, weights, traction, massage and other as indicated.
• Supervise patients in the completion of their exercise and training program to maximize their functional independence.
• Progress the patient in treatment as indicated by the patient's progress in therapy and the treatment plan.
• Respect the patient's right to privacy and confidentiality.
