This is advanced-level work performing or supervising a variety of completes tasks under limited supervision where general policies may not have been previously established and standard methods may not be available. The job requires extensive knowledge of counseling techniques and social work principles and practices. Incumbents may use skill in developing methods and procedures for special applications or functions.

Program Development and Implementation, Evaluation and Data Collection:

  • Develop, coordinate, and deliver community-wide health education initiatives focused on the Children’s Medical Home priority across diverse community and clinical settings.
  • Plan and implement evidence-based educational workshops in schools, community centers, healthcare facilities, and public health events to help families establish and maintain a medical home for their child(ren).
  • Create culturally and linguistically appropriate educational materials and strategies using best-practice models and data-driven approaches.
  • Conduct surveys with families to determine whether their child meets medical home criteria, including having a regular provider, receiving necessary referrals and obtaining coordinated care.
  • Document grant outcomes and assist in evaluating grant priority effectiveness using collected data.
  • Identify trends, barriers, and community needs to enhance grant reach and improve health outcomes for children.
  • Submit Children’s Medical Home data bi-annually in alignment with Healthy Babies Workplan requirements.

Outreach, Community Engagement and Collaboration:

  • Conduct targeted outreach at community events, county health departments, and partner organizations to increase awareness of the benefits of establishing a medical home for children.
  • Serve as a liaison to mobilize clinicians, community partners, and stakeholders to support and participate in medical home initiatives.
  • Build and maintain partnerships with clinicians, local organizations, schools, and community groups to strengthen grant impact and delivery.
  • Participate in interdisciplinary meetings and collaborative workgroups to align efforts, share progress, and support integrated community health strategies.

 Public Health Competency Domain 1: Analytical/Assessment Skills

Family Planning Quality Improvement and Training:

  • Ensure clinical staff compliance with Title X requirements and corrective actions by implementing onsite training, quarterly training, and other Family Planning training as directed by the Family Planning Program Office.
  • Provide oversight of Duval’s compliance with required Title X Family Planning training, ensuring timely completion and accurate documentation in the Family Planning Program spreadsheet.
  • Review staff training records and documentation to identify training gaps and serve as the point of contact for planning and implementing supplemental clinical and community training activities that support family planning service delivery.
