 We are looking for a Home Care Aide to join our team! You will be directly working with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care, but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role.

RESPONSIBILITIES:

• Maintain records of patient care, conditions, progress or problems to report and discuss observations with supervisor
•  Engage patients in exercises or other activities 
• Medication reminder, assist with light housekeeping, meal prep. errands & ADL's
• Assist with transferring (mobility) and with Bed pan exp. required
• Must have Inpatient experience: Preferably a CNA, Medical Assistant or Certified Home Health Aide

This position is part-time for Weekends Only (with possible increase in hours) (12:00pm to 5pm) must assist the patient with using a bedpan. 
The overnight shift (11:45pm to 5:45am) must prepare both breakfast and lunch for the patient
