About the role

Work Shift:

Day - 8 Hour (United States of America)Monday-Friday / 8:00am - 5:00pm (40 Weekly Hours)

The Wellness Nurse will perform annual wellness visits for patients of Houston Family Care at Pavilion to include all insured and uninsured patients, with focus on CMS annual regulations. Serves as a resource to the Medical staff on utilization and regulatory issues and participates in coordination of individualized patient care services. Collaborates with other members of the healthcare team to monitor and evaluate patient annual wellness goals and expectations, and provide educational counseling to obtain those goals. Ensures proper documentation is entered into the clinical practice management system,
and or other systems as required by CMS. Responsible for following all departments’ documentation standards within the patient chart. Will maintain close communication with all physicians, clinical and administration staff of the practice. Will maintain confidentiality of all patient information. Adhere to all health and safety regulations within the organization.

Education & Training:
Graduate of an accredited School of Nursing.

Experience:
Three (3) years nursing experience.

Required Certification/Registration/Licensure:
Current US state RN license with authorization to practice in the state of GA.
Current AHA/ARC BLS certification. AHA ACLS/ARC ALS and AHA/ARC
PALS certification required or obtained within six (6) months of hire.

Knowledge, Skills & Abilities:
Must be able to utilize diagnostic equipment as well as patient care equipment. Must be able to utilize personal protective equipment such as gloves, gown, mask, etc. Proficient in utilizing Microsoft Office (i.e., Excel, Word, PowerPoint, Outlook, etc.). Proficiency in Intranet/Internet use. Excellent interpersonal and oral/written communication skills. Skilled in conflict resolution, critical thinking, task management and appropriate prioritization/delegation.
