Job Summary: The Triage Registered Nurse (RN) is responsible for providing nursing services to assigned patients. RNs provide direct patient care, collaborate with the other Interdisciplinary Team (IDT) members within the care team, and with other health care providers in various care settings. Patient care provided by RNs is done in a manner that promotes an interdisciplinary team approach, respects patients/families goals for care, meets Medicare/Medicaid conditions of participation and accreditation requirements and reflects the mission and philosophy of TRU Community Care. Upon request, participates in all areas of clinical services.
Duties and Responsibilities:
• Comprehensively assesses and anticipates all aspects of the patient’s clinical nursing needs, pain and other symptoms and, in collaboration with primary care physician or hospice physician, develops an individualized plan of care PRN.
• Incorporates patient/family preferences and goals within the plan of care
• Supports and respects the patient’s and family’s unique diversity, including spiritual and cultural beliefs
• Completes documentation in an accurate and complete manner as per agency standard and in compliance with regulatory standards
• Provides patient and family education as appropriate; areas of education include, but are not limited to personal care, medications, procedures, safety, diet and nutrition, disease process, death and dying
• Contributes to the team, utilizes team resources, supports team decisions, and maintains positive working relationships with all team members
• Works collaboratively with volunteers and provides ongoing support and guidance
• Demonstrates effective verbal, written, and electronic communication skills; gives and receives feedback in a timely open manner; engages collaboratively in conflict resolution processes as needed
• Is flexible and responsive in meeting and exceeding the expectations of all customers, both internal and external
• Participates in mandatory educational activities and required meetings
• Is competent using agency information and technology systems
• The Triage Registered Nurses performs in supportive role to the Hospice Care Team – Case Managers, answering calls coming into the agency with response to the telephone call by either delegating tasks and or going out on a Home Care Visit
• The Triage Registered Nurses performs in supportive role to the Palliative Case Managers, answering calls coming into the agency with response to the telephone call
• All other duties as assigned
Qualifications and Requirements:
Education:
• Current RN license in the state of California
• BSN or higher degree preferred
Experience:
• Must have a minimum of one year RN experience – hospice or home health experience preferred
• Experience as telephone triage nurse preferred
Qualifications:
• Requires travel to multiple work locations
• Must comply with agency health screening and immunization requirements
• Must possess CPR certification or be able to obtain within 60 days of hire.
• Must be able to demonstrate computer competency
