In this position, the ACM is responsible for assisting with writing letters and completing forms at the request of providers/therapists.

  

 

 

 

The Administrative Case Manager assumes responsibility for assuring that residents in the defined geographic area receive appropriate and adequate mental health services and community support. Through screening, referral, and short-term/low intensity assistance, the incumbent is responsible for assisting individuals with mental illnesses in accessing federal and state financial entitlements, in coordinating treatment/rehabilitation services, and in linking to/acquiring community supports and resources needed to meet daily living/health needs. Not all administrative case managers are responsible for all position requirements.

JOB DUTIES AND RESPONSIBILITIES:

  • Assist service participants in applying for Medical Assistance, Supplemental Security Income (SSI), Social Security Disability (SSD), pharmaceutical Patient Assistance Programs, low-income housing and other resources/benefits as needed.

  • Function as a liaison for service participants: Interested in referral to and participating in educational/vocational services offered locally; Receiving treatment at area state and community hospitals. Visit persons from defined geographic areas who are hospitalized, attend treatment planning meetings, and participate in discharge planning processes per county policy; Incarcerated at correctional facilities; assist in planning to ensure appropriate community support is in place prior to release.

  • Monitor treatment adherence of participants on involuntary outpatient commitments (IPOC). Submit IOPC admission and termination information to appropriate county personnel as required.   • Maintain community resource files and provide information and referral services to service participants, families and members of surrounding communities.

  • Attend scheduled meetings as required/needed.

  • Complete documentation of services provided.

  • Maintains confidentiality of all materials handled within the Network/ Entity as well as the proper release of information. 

  • Complies with Network and departmental policies regarding issues of employee, patient and environmental safety and follows appropriate reporting requirements. 

  • Demonstrates/models the Network’s PCRAFT values during interactions with all customers.   

  • Demonstrates Performance Improvement in the following areas as appropriate: Clinical Care/Outcomes, Customer/Service Improvement, Operational System/Process, and Safety.

  • Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, processes and practices.

  • Complies with Network and departmental policies regarding attendance and dress code. 

  • Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated.

  • Other duties as assigned.

PHYSICAL AND SENSORY REQUIREMENTS:

Sitting up to 7 hours per day; 1 hour at a time. Stand for up to 6 total hours per day and up to 4 total consecutive hours.  Must be able to perceive attributes of an object through touch. Must have the ability for hearing as it relates to normal conversation, high frequency, and low frequency hearing sounds. Must have the ability for seeing as it relates to general vision, near vision, color vision and peripheral vision.


EDUCATION:

Required: A high school diploma with one year experience working in a public or private human service organization.
