Registered Nurse (RN) Patient Care Navigator - Spine

Inova Fairfax Hospital is looking for a dedicated Registered Nurse (RN) Patient Care Navigator - Spine to join the team. This role will be full-time, M-F, regular office hours (must be flexible with start/end times depending on department needs). In-person – no weekends, no nights, no on-call.

Registered Nurse (RN) Patient Care Navigator - Spine Job Responsibilities:

• Guides patient and family through the health system from diagnosis, testing, treatment and follow-up care to assist patients with navigating the continuum of care. 
• Coordinates a patient's plan of care which may include appointment scheduling, confirming patient's attendance, transportation, assisting with referral process, and answering patient questions. 
• Acts as a liaison, advocate and point of contact to patients in need of services such as screening, diagnostic treatment, clinical research participation, support services, and follow-up/aftercare plan by helping them access the best healthcare outcomes from doctors, hospitals and others who provide their required services. 
• Manages complex cases effectively and efficiently. 
• Creates a seamless flow through the health system to facilitate the patient's care. 
• Collaborates with multidisciplinary teams to address patient questions and responses about the care process and treatment options.
• Communicates with payers to facilitate covered day reimbursement certification for assigned patients. Discusses payer criteria and issues on a case-by-case basis with clinical staff and follows up to resolve problems with payors as needed.
• Performs other duties as assigned.

Registered Nurse (RN) Patient Care Navigator - Spine Additional Requirements:

• Work schedule: full-time, M-F, regular office hours (must be flexible with start/end times depending on department needs). In-person – no weekends, no nights, no on-call.
• Education: Bachelors Degree in Nursing
• Experience: 3 years of nursing
• Certification: Basic Life Support from the American Heart Association; Licensed in the Commonwealth of Virginia as a Registered Nurse.

Registered Nurse (RN) Patient Care Navigator - Spine Preferred Qualifications: 

• Experience: Prior experience working with spine patients; experience as a Navigator/Coordinator of surgical services
• Other: Comfortability working with a multi-team system (different team members, units, providers, etc.); can form strong relationships with the care teams and patients
