Job Description

The Clinical Pharmacist oversees the medication distribution system for their area of responsibility. In addition, the Clinical Pharmacist identifies, prevents, and resolves drug therapy problems by ensuring each medication is indicated, effective, safe, and convenient for the patient.Knowledge of pharmacokinetic principles required for medication dosingMonitor patients for response to medicationsIdentify and document sentinel/serious events, near misses, medication errors and adverse drug reactions in the electronic reporting systemPromote and assure compliance with the hospital formulary and policies and procedures defined by the P T Committee.Provide care that is appropriate to the chronological age, developmental age, social maturity, and cultural background of the patient Education: Doctor of Pharmacy or RPHExperience: PGY1 Pharmacy Residency preferred (consider: 5 years of experience + RPH acceptable for education)Pharmacist License in the State of Pennsylvania
