LIC INDEP CLINICAL SOCIAL WORKER

Primary Scheduled Hours: Days, 80 hours per pay period

 

Job Summary: The Behavioral Health Therapist, as a part of a multidisciplinary team, will provide Behavioral Health services including initial evaluations, individual and/or group therapeutic services, assessments, treatment plans, and discharge planning. In addition, the Behavioral Health Therapist will provide behavioral health consultation services. Clinical services will focus on time-limited, evidence-based therapy with patients and families that addresses psychological, psychosocial, environmental, age-specific, and cultural issues to maximize social, emotional, and physical well-being.

 

Essential Job Functions:

• Screen and assess patients for common behavioral health and substance use disorders. Facilitate patient engagement and follow-up care
• Provide both brief and full-length behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, cognitive behavioral therapy, or other treatments as appropriate.
• Support the mental and physical health care of patients on an assigned patient caseload. Maintain effective and professional relationships with patients and other members of the care team. Closely coordinate care with the patient’s medical provider and, when appropriate, other behavioral health providers.
• Work with NHHS medical providers and psychiatric consultants to seek ways to develop integrated health and BH-SUD care delivery systems to reduce BH-SUD emergency/crisis utilization and hospital
• Work with chronically ill and/or high-risk patients, their families/caregivers, and clinic, hospital, residential care, social services, and community resources in a team
• Monitor care and service delivery by auditing patient medical records for progress toward desired outcomes, intervening to overcome deviations in expected care plans, and maintaining ongoing communication with care providers/partners regarding variances from the care delivery
• In conjunction with providers, assist in the development of programs to increase a patient’s ability for self-management/shared decision making; assist to provide education/information related to treatments, procedures, and

• Effectively engage patients in a therapeutic relationship, as
• Create and promote adherence to a care plan or goal plan (developed in coordination with the patient, primary care provider, and family/caregiver), and interacts with involved patient

 

Education Background and Experience Requirements

Minimum Education:

Must have specialized training and/or meet clinical licensure requirements to provide evidence-based psychosocial interventions, such as problem-solving treatment and cognitive behavioral therapy. Licensures and/or educational preparation include:

·         Licensed Independent Clinical Social Worker

·         Licensed Psychologist

Required Experience:

·         Evidence of communication, organization, and counseling skills

·         Experience with health information technology systems

·         Basic knowledge of psychopharmacology for common behavioral health disorders

·         Working knowledge of differential diagnoses of common behavioral health and/or substance use disorders

·         Working knowledge of evidence-based psychosocial treatments and behavioral interventions for common behavioral health disorders (e.g.- Cognitive Behavioral Therapy/CBT, motivational interviewing, problem-solving treatment, behavioral activation)

·         Ability to work with providers and other members of the healthcare team in a collaborative manner

·         Knowledge of Medicare, Medicaid, third-party payers, Behavioral Health-Substance Use Disorder (BH-SUD) coding

Preferred Qualifications:

Experience with Epic electronic health record system
