Job Summary:

Provides direct and consultative Occupational Therapy services to home care clients. The employee exercises professional judgment in the provision of Occupational Therapy services in the Home Health Program. In selected cases, the Occupational Therapist functions as Case Manager, using the development of an interdisciplinary plan of care and manages resources to achieve identified outcomes.

This is a casual position. Casual employees work on an ‘as needed’ basis to meet the needs of the business. Workdays/hours will often vary from week to week.

Work schedule would be between the hours of 8:00AM - 4:30PM, weekdays. No evenings, weekends, or on-call requirements.

Responsibilities:

Essential Functions

Provides an accurate and thorough assessment of the patient/family to identify and plan for the needs of an individual.

• Assesses occupational therapy needs of client on admission and periodically during care. Evaluates client progress toward goal achievement, outcomes and effectiveness of client care.
• In the case management role, manages assigned caseload, assesses client needs and develops an appropriate referral to specialty teams and internal/external resources.
• Consults and educates clients, agency staff and clients; families regarding adaptations of function or environment to enhance independence for clients, the disease process, self care techniques and prevention strategies.
•  Performs safe and effective skilled clinical procedures and techniques.

Implementation of a Occupational therapy treatment regimen.

• Provides Occupational Therapy services to home care and hospice clients/families, designed to increase strength, coordination, range of motion and function of affected extremities and to retain damaged nerve pathways or to teach compensation utilizing nerves which remain; collaborates with other involved disciplines to develop, implement, coordinate and evaluate the total care plan for the client/family.
• Teaches skills necessary for maximal attainable independence to clients, family members, caretakers and other agency staff including environmental adaptation and specific homemaking and activities of daily living skills.
• Designs and Fabricates devices to increase independence for the physically impaired; fabricates splints to correct or prevent deformities.
• Provides evaluation and training for perceptual disorders.
• Coordinate and supervises patient care to provide the management of a multidisciplinary approach to patient care.
  • Actively participates in a multi disciplinary team providing patient-focused care, assessing and evaluating client progress toward achievement, outcomes and effectiveness of care; revises plan ass appropriate on consultation with client's physician.
  • Informs Physician and other health team members of changes in clients needs.
  • Works with client to obtain equipment to increase client's function and independence.

Evaluation of the plan of care and treatment for all patients under his/her care to promote resolution of patient problems and positive patient outcomes.

• Assess and evaluates client progress toward goal achievement, outcomes and effectiveness of client of client care.
• Works with clients in planning for discharge from Occupational Therapy services.

Supports agency financial and quality goals.

• Proper completion and submission of required documentation within 24 hours of services provided.
• Meets all of the productivity standards as identified by the agency.
• Participates in the management of the Case Mix Index, Low Utilization Payment Adjustment and Missed Therapy Resource Utilization.
• Assesses client's financial resources and establishes the appropriate funding source and billing sequence. Collaborates with office staff regarding coverage and authorizations.
