Responsibilities:

• ·       Provide one-on-one patient care in the patient's home, following a detailed care plan provided by the Nurse Care Manager and/or Physician
• ·       Perform skilled nursing tasks such as administering medications, wound care, and monitoring vital signs
• ·      Collaborate with the patient's healthcare team to develop and implement an effective care plan
• ·      Educate the patient and their family on proper care and treatment method
• ·      Maintain accurate and up-to-date patient records
• ·      Communicate effectively with patients, families, and healthcare professionals

Qualifications:

• ·      Registered Nurse (RN) license in IL
• ·      Minimum of 2 years of nursing experience, preferably in home health or hospice care
• ·       BLS/CPR certification
• ·      Strong clinical skills and knowledge of medical equipment and procedures
• ·      Excellent communication and interpersonal skills
• ·      Compassionate, patient-centered approach to caregiving
• ·      Ability to work independently and manage time effectively
