Your Responsibilities

• Deliver skilled nursing care in accordance with state regulations and professional standards.
• Perform client care under the supervision of a Registered Nurse, following care plans and policies.
• Support patients in achieving improved health outcomes through short home visits.
• Ensure compliance with agency standards for quality and safety.

What You Need to Succeed

• Current LPN License (as required by the state).
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing. OR Graduate of an accredited Certificate, Diploma. 
• 1+ year of experience in an acute care or equivalent setting.
• CPR Certification (American Heart Association or American Red Cross).
• A positive attitude and dedication to delivering quality patient care.
• Strong communication skills and reliable transportation.
• Ability to sit, stand, bend, lift, and move intermittently, with the capability to lift 50-100 lbs.
