Home Health Physical Therapist (PRN)

AdvisaCare is seeking a friendly and experienced Home Health Physical Therapist to join our team on a PRN (as needed) basis! Our ideal candidate will have a passion for helping individuals achieve their physical goals while maintaining their independence in their home environment. As a member of our team, you will provide exceptional customer service while delivering personalized care to each of our clients.

Responsibilities

• Conduct comprehensive evaluations of the client's physical status and create a personalized treatment plan.
• Administer physical therapy treatments and exercises to clients in their home environment.
• Monitor and record progress towards achieving physical therapy goals and adjust treatment plan as needed.
• Educate clients and their families on proper techniques to maximize physical functioning and prevent injury.
• Communicate and collaborate with other members of the healthcare team to ensure comprehensive care for the client.
• Document all visits and treatments accurately and in a timely manner.
• Maintain confidentiality and comply with all HIPAA regulations.

Requirements

• Current state licensure as a Physical Therapist in Michigan.
• Valid driver's license and reliable transportation.
• Ability to communicate effectively with clients, families, and team members.
• Strong time management and organizational skills.
• Ability to work independently and make sound clinical decisions.
• Basic computer skills and experience with electronic medical records.
