Job Role Summary
Works closely with the Bureau of Environmental Health/Housing & Neighborhood Health Department and other community service providers to maximize health outcomes for individuals, families, and communities. The majority of services are home-based. Responsible for providing a continuum of social work services which will assist patients/clients and their families to find practical ways of resolving social, emotional and environmental problems that interfere with optimum functioning and attainment of total health.
Associated Job Duties
- Provides therapeutic social work intervention to a diverse client system which may include but not limited to psychosocial assessment, short/long term counseling, crisis intervention, community resource referrals, client advocacy, support, outreach, follow-up, and intra/inter agency consultation, specifically in conjunction with the Bureau of Environmental Health
- Initiates social work plans for patients/clients through professional collaboration with health care personnel and other external agencies, utilizing screening questionnaires, diagnostic assessment and individualized treatment plans
- Provides services to patients/clients who demonstrate a need for assistance with psychosocial/emotional/mental health issues, health care, adult/child abuse protection, domestic violence, health insurance, food, clothing, dental assistance, with primary focus being on housing/environmental/shelter problems, impacting their ability to have and or maintain a healthy lifestyle
- Provides direction in the selection, analysis, summarization and presentation of patient/client and related information for service delivery planning, case conferences and professional consultation as it relates to housing and neighborhood health issues and other psychosocial issues, environmental, economic, etc...
- Facilitates the coordination and implementation of services which will assure appropriate services and continuity of care for patients, clients, and their families of Marion County, who have specific housing and environmental related concerns
- Prepares written reports and clinical records (psychosocial assessments, progress notes, summaries, statistical forms, etc.) of services rendered; responds timely to requests for information
- Assumes advocacy/liaison/collaborative role for clients/patients via contacts with community agencies/personnel, hospital personnel, court disposition hearings, abuse cases, etc; collaborates with inter/intra agency personnel in referral activities
- Maintains a current file of referral sources; cultivates a network of resources and collaborates with inter/intra agency personnel involving referral activities
- Patients/Clients and their families’ records and information, staff records of all other confidential information presented orally in writing will be kept confidential
- Maintain confidentiality of patient/client records and information in accordance with HIPAA guidelines
- Assumes responsibility for maintaining current knowledge of information/counseling techniques relevant to specialty area

- Functions as a skilled consultant and advocate, concerning effective management of interpersonal situations which affect the positive coping skills of individuals, families, as it relates issues including housing; and involving active collaboration within a multidisciplinary team
