Madison County Memorial Hospital (MCMH) is a critical access hospital. It is also a hospital district facility, which is a quasi-government hospital whose board is appointed by the Governor of Florida. It operates twenty-five inpatient beds. MCMH generates approximately $19,000,000 in revenue each year and employs about 100 full-time staff.
MCMH is located about five miles north of Interstate-10, approximately 50 miles east of Tallahassee, Florida. Madison is the county seat of a county with a population of approximately 20,000. The county’s economic base is agriculture, State correctional facility, school district, local government and small retail.
Job Description
The Case Manager (CM) is responsible for the development and implementation of effective and on-going Case Management Utilization Review, Discharge Planning Process, and assisting with Quality Improvement Initiative Projects and Quality Improvement/Clinical Education programs; Responsible for the efficient and competent case management of observations, inpatient and swingbed patients. Develops policies and procedures for areas of responsibility as needed.
The CM is also responsible for filling in vacant positions that fall within the nurse practice act for registered nurses/ licensed practical nurses and will rotate with other nurses and management team members as on-call nurse to assist and cover the needs of all nursing departments.
The CM is responsible for the timely review and submission of Observation charges.
The CM is responsible for excellent communication and management of resources and upholding excellent clinical standards of care.
Qualifications
- A Florida licensed Registered Nurse
- Minimum 2 years acute care nursing or skilled nursing facility experience
- One year Utilization Review/Case Management/ insurance verification /authorization Experience required
- Certification in Case Management preferred
- Current CPR, ACLS and PALS certification (ACLS within three months of hire date and PALS within six months)
- Excellent written, computer, and verbal communication skills with a strong ability to comprehend and follow provider orders for safe patient
- Excellent interpersonal skills with the ability to interact effectively and professionally with people at all socio-economic and organizational levels

- Excellent internal and external customer service skills
