About the Role:

As the Director of Healthcare Social Services, you will lead and coordinate our social services team while promoting dignity, self-determination, and a high quality of life for residents. This role is both leadership-focused and hands-on, providing direct support to residents and overseeing critical functions such as behavior planning, care coordination, and transitional services.

Key Responsibilities:

• Provide direct supervision for the Social Services team including scheduling, coaching, and performance evaluations.
• Serve as primary social worker for designated units.
• Lead Behavior Team and Care Planning meetings, ensuring follow-up and interdisciplinary coordination.
• Collaborate with Nursing and Resident Services Coordinators for smooth transitions across care settings.
• Serve as a backup for Admissions and participate in on-call rotation.

What We’re Looking For:

• Bachelor’s degree in Social Work, Sociology, Psychology, or related field (Master’s preferred).
• 5+ years of long-term care experience.
• 3–5 years of supervisory experience.
• Familiarity with Medicare, Medicaid, VA, and pre-admission screening.
• Current or eligible for Social Work licensure.

Schedule:

• Day shift: Monday–Friday
• Some weekend hours & management rotation required
