POSITION SUMMARY:

Responsible for providing direct services as a clinician on the team; assisting with providing supervision to each specialty role based on evidence-based practice, trauma-informed care, cultural humility, and person-centered language; and providing administrative oversight to the team in conjunction with team leader. The Asst Team Leader regularly conducts individual supervision, which may include direction to staff regarding individual cases or side-by-side contact with staff. The assistant team leader will be supervised by Team Leader in consultation with prescriber.

Essential Accountabilities

  1. Co-Leads daily team organizational and service planning meetings, provides direction to staff regarding individual cases, and regularly conducts individual and group supervision meetings. 
  2. Meets required productivity targets and completes all documentation within assigned time frames.
  3. Monitors and coaches staff on meeting productivity measures.
  4. Directs and coordinates the client admission process and treatment, rehabilitation and support services of the program. 
  5. Participate in staff recruitment, interviewing, hiring, work assignments and orientation and performance supervision.
  6. Develop and implement staff orientation and training.
  7. Develop and maintain program policies and procedures and revise as necessary.
  8. Perform on-call duty for crisis intervention and for staff supervision and consultation. 
  9. Oversee and monitor accurate use of internal electronic health record (Avatar).
  10. Provide guidance in engagement and communication with enrollees.                                                             
  11. Provide leadership through positive modeling and be open to the opinions of all team members.
  12. Provide guidance to ACT staff for identification of potential barriers to care and resolutions to those barriers.
  13. Audit client charts on a regular basis.
  14. Coordinate and monitor services with AOT, mental health and criminal courts, probation, and child protection services on an ongoing basis.
  15. Responsible for crisis intervention requiring 24 hours on call availability.
  16. Provide disciplinary actions to staff in violation of agency, contract or program policy.
  17. Act as a clinical liaison between the team and outside entities.
  18. Document any progress notes in EHR within 48 hours of all client and collateral contacts.
  19. Ensure all documentation is completed in a timely fashion and remain in compliance with agency, OMH, and AOT regulations.
  20. Communicate pertinent consumer and operational information to the Deputy Director, Division Director and Regulatory bodies.
  21. Participate/Organize case conferences and case consultation to ensure a high standard of service provision.
  22. Conduct field based visits. The ACT Program provides 85% of their services in the community.
  23. Participate in OMH approved/required ACT Orientations and Trainings.
  24. Input all data into Vellohealth software application and monitor that all staff are keeping application up to date.
