Manager, Clinical Services
The Clinical Services Manager will oversee and have responsibility for the Care Coordinator and Strive Home Visit program. This role will report to the VP of Central Clinical Operations and will be the owner and subject matter expert of all Care Coordinator (CC) workflows and home visit processes, metrics, and outcomes. They will be responsible for all onboarding/training of new CC staff after they complete general New Hire Orientation (NHO). In addition, this role will assist with auditing and coaching CCs who are not meeting audit thresholds. The CC Program Manager will work closely with the market teams to support rollout and reporting on new workflows, in addition to coordinating the education on training materials and updates to workflows.
Essential Functions
- Support the development of CC programs and home visit programs including the deployment and adoption of CC workflows and processes in the markets
- Assist with onboarding new staff, which may include working with the Education team to develop an onboarding program for CC and home visit roles
- Support quality audits in the markets by completing audits monthly and providing regular coaching and support back to the CCs
- Work with the Education team on the development of educational materials around new workflows and processes
- Function as the subject matter expert on CC and home visit processes, workflows, and scope
- Work closely with Licensed Social Worker (LSW) Lead to develop strategies and workflows that focus on Social Determinants of Health support for our patients
- Manage and monitor reporting and dashboards for CC role and home visit program
- Manage ordering home visit supplies for new hires performing home visits
- Other duties as assigned
Minimum Qualifications
- 3 years supervisory/management experience
- Bachelor’s degree
- Graduate of a state-approved certification program CHW, MA, or current, unrestricted license as an LPN/LVN in the state(s) of practice, or experience as an office manager in a provider practice or RN with experience overseeing CHW or CCs at Health Plan or CM organization
- 4+ years of experience in a professional office environment
Preferred Qualifications
- Managed/Value-Based Care and payor experience preferred.
- Experience performing home visits
- Ability to build credible and effective relationships across a matrixed environment.
- Strong analytical skills.
- Strong project management skills.
- Excellent communication and interpersonal skills.
- Demonstrated leadership capabilities.
- Customer service experience
- Intermediate proficiency in MS Word, Excel, PowerPoint, and Outlook
- Excellent verbal and written communication skills
- Skilled at dealing with confidential information and/or issues using discretion and judgment
- Communicates clearly, respectfully, and thoughtfully
Annual Salary Range: $83,700.00-$101,500.00
