YOUR TOP RESPONSIBILITIES

  • You will be responsible for conducting case management assessments on community and program clients to determine their fitness for our program.
  • You will assist in developing individual case plans for each client and for evaluating progress along those case plans.
  • You will maintain and supervise client’s self-administered prescriptions to ensure that are followed as written.
  • You will coordinate clients’ schedules for appointments, including medical, legal, and counseling, and may provide transportation to off-campus appointments.
  • You will offer administrative support to the Campus Director or clinical staff.
  • You will lead or participate in devotions, prayers, or other organization activities that promote a credible Christian witness and lifestyle.

Requirements

WHAT YOU SHOULD HAVE

  • High school diploma or GED.
  • 1 year of experience performing case management in a similar setting.
  • Certification in Peer Support highly desired.
  • You have excellent composure in all situations.
  • You are discreet and understand the importance of keeping client information confidential.
  • You have strong communication skills.
  • You are proficient in the Microsoft Office suite of applications.
  • You have a solid and credible Christian witness and lifestyle that exemplifies Christ in character and actions.
