What you’ll do:

Patient Education: Provide comprehensive education to patients undergoing joint replacement procedures and their families, covering pre-operative preparation, surgical procedures, post-operative care, and rehabilitation.

Care Coordination: Serve as the primary point of contact for joint replacement patients, coordinating all aspects of their care from pre-operative assessments to post-operative follow-up appointments.

Resource Referral: Identify and coordinate additional resources and support services for joint replacement patients, including physical therapy, occupational therapy, home health services, and durable medical equipment.

Patient Monitoring: Monitor patient progress throughout the joint replacement journey, including pre-operative optimization, surgical recovery, and rehabilitation phases. Provide ongoing support and guidance to facilitate optimal outcomes.

Documentation and Reporting: Maintain accurate and up-to-date records of patient assessments, care plans, and coordination activities. Generate reports and analyze data to track patient outcomes and identify opportunities for improvement.

Community Engagement: Collaborate with community organizations, support groups, and other healthcare providers to enhance the continuity of care for joint replacement patients and promote community resources for rehabilitation and recovery.

High school diploma or equivalent required

Previous experience in medical practices with care coordination.

Excellent communication, interpersonal, and patient education skills.

Ability to work collaboratively as part of a multidisciplinary team, including surgeons, nurses, therapists, and support staff.

Strong organizational skills and attention to detail, with the ability to manage multiple tasks and priorities effectively.

Proficiency in using electronic health records (EHR) and other healthcare software systems.
