Responsibilities
- Obtain and document vital signs including; head circumference, pulse, respiration rate, height and weight, blood pressure, temperature; collect specimens for various labs including performing veipuncture; perform vision and hearing screening.
- Assist MD and/or PNP in provision of physical examinations
- Ensure the medical records contain written documentation of all health and mental health services received.
- Follow up on appointments and referrals for specialists for a specified caseload
- Completion of camp and school medical forms for specified caseloads
- Track and monitor required health visits for specified caseloads
- Data entry of health information into electronic systems as required
- Ensure that documentation of recommended health visits that are obtained in community based settings are received and stored into medical record of specified caseload
- Assist in quality assurance activities involving services provided to children in assigned caseload
- Attend case conferences and Clinical Planning Conferences as assigned
- Attend Service Plan Review meetings when needed to provide health related information, which is germane to developing and implementing permanency plan for children in assigned caseload.

Qualifications
Other Requirements:
- 2 years clinical experience in pediatric clinic or physicians office
- Computer skills
- Knowledge of or experience in foster care
- Knowledge of and/or experience in management of medical records
- Knowledge of and/or experience in health care case management
- CPR Certification
- Good written and verbal communication skills
- Certified to perform pre and post HIV test counseling
- Spanish speaking a plus
- Demonstrated strong commitment to safety.
Salary Range:
$25.46/hour (plus $1.92 for Language (English/Spanish) additive and/or $2.09 Education Additive)Apply for this role
