Description

Provide coordination and supervision of the daily implementation of the Individual specific Plan. Create teaching and support strategies as part of the team process to meet outcomes and action steps. To be an advocate for the individual and ensure all health and safety needs are met with dignity. Provide mentorship, guidance, and training to employees. Work as a direct liaison to the Program Director in all aspects of the role including budgetary allotments/utilizations, TSS changes, state required exemptions, internal/external conflict resolution, management of Supported and Family Living coordination employees/contractors, ongoing service improvements through new ideas and implementations, billing review and approvals, ensure staffing needs are met to successfully provide a superior service.

Requirements

. Review written and verbal reports/documentation related to daily progress and provide intervention or training to the service coordinators and homes teams as needed.

2. Advocate and teach others to advocate for the individual supported.

3. Provide emotional support and feedback to the individual and their family if applicable.

4. Act as a positive role model to both individuals and employees.

5. Provide oversite for all supported and family living service implementations including medication management, medical continuity, ISP implementation, transportation needs, community involvement/access, nutrition and a safe warm and inviting home environment.

6. Provide oversite for all employee-based needs and functions including staffing ratios, scheduling, training, assessment, evaluations, infractions, terminations. Provide necessary ongoing contract oversite including contract reviews and cancellations.

7. Evaluate employee’s/contractors' ability to successfully meet the needs of the individual. Provide formal and informal feedback/evaluations and training to the employee/contractor.

8. Evaluate service coordination and the team's ability to successfully meet the needs of the individual. Provide formal and informal feedback/evaluations and training to the employee/contractors.

9. Lead the QA/QI committee, including data collection, correlation, and analysis of data.

10. Complete all required training including the DDSD service coordination requirements and work within all HeartWell Policies & Procedures.

11. Maintain regular communication with the assigned nurse, service coordinator, program director, and HR on all appropriate related matters.

12. Conduct regular audits of all coordinator’s caseloads, documentation, Therap data, and online storage.

13. Conduct monthly clinicals with coordination, identify deficiencies and strengths, report to program director. Follow up to ensure compliance with all standards.

14. Active preparation for and participation in all internal and external audits/surveys

15. Actively Mentor and train employees within the role they currently hold as well as provide advancement opportunity through supporting the management training program.

16. Work with the incident/education coordinator to support training of staff, identifying needs for increased training or training improvements for employees.

17. Attend initial meet and greets, transition meetings, advise coordination, HR, Program Director of needed service provisions and staffing requirements.

Additional direct tasks

· On-call for home and agency in accordance with current guidelines

· Utilize and manage agency petty cash card for location purchasing needs

· Ensure management of day-to-day staffing needs.

· Bi-weekly/monthly documentation verification including billing verification and submission.

·· Provide training for service coordination

· Verify new hire training and competencies

· Verify medical appointments and follow-ups

· Verify monthly location safety including safety drill completion

· Verify nutritional recommendations are met per individual plans

· Verify client Funds reconciliation (ledger / receipts)

· Verify/complete monthly activity schedules.

· Communicate daily with service coordinators for updates and observations

· Service coordination time sheet review and submittal including review of locations working schedule

· Verify monthly reports/documentation review

· Ensure team driven monthly emails.

· Verify monthly updates to annual/semiannual ISP reporting

· Review TSS frequency - work with teams for accuracy and changes as necessary

· Teach Therap GER process to employees/contractors and support the ANE reporting process

· Enforce RORA process
