This position represents an amazing opportunity for a nurse practitioner (APRN)  with a big heart and entrepreneurial spirit who is passionate about chronic disease management and optimizing care delivery for seniors.

   

In this role you will be working in a next generation virtual Responsive Care central services clinic that dramatically expands access to medications for America's most vulnerable seniors and supports all of the medication renewal and results management functions for Devoted Medical primary care.  

The clinic focuses on three things: 

 

Optimizing medication adherence for Devoted Health members with hypertension, hyperlipidemia and diabetes who are at risk of running out of their medications.

Providing rapid response time for the high volume of renewals generated by Devoted Medical Primary Care and other longitudinal care medical services

Facilitating efficient results management on behalf of  Devoted Medical Primary Care.

 

You will serve as the prescriber in our medication renewal service that we call “Medications on Time” and will also work closely with our clinical pharmacy guide time that comprises over 90 pharmacy technicians who reach out and support members who are approaching or have become nonadherent to their medications. You will also manage the medication renewal queue and results inbox for our primary care service lines. You will utilize and help improve our home-grown technology and electronic health information platform to carry out virtual visits as well as managing asynchronous prescription refills for patients.  

Responsibilities will include:

  • Manage a refill queue for patients enrolled in our primary care service and our Medications on Time services. 

  • Perform virtual visits for members in whom additional medical evaluation is required to safely renew their medications. 

  • Accurately diagnose and manage chronic conditions. Identify and evaluate risk factors, comorbidities and possible contraindications for treatment. Provide counseling on medication adherence, potential side effects.

  • Collaborate closely with other members of the care team including PCPs, endocrinologists, cardiologists and other Devoted team members including pharmacy, clinical nursing, social work. Interface with family members and caregivers as needed  to coordinate  holistic care for the member, to ensure continuity of care and deliver a collaborative care plan.

  • Utilize our home grown electronic health information system for visits while also providing feedback on how to improve the interface. 

  • Maintain accurate and up-to-date patient medical records, ensuring compliance with relevant legal and ethical guidelines.

  • Participate in quality improvement initiatives  and ongoing professional development to stay current on best practices and advancements in cardiometabolic care.

  • Adhere to all relevant laws, regulations, and industry standards, including patient privacy and telehealth regulations.
