General Description:
Under minimal direction, plans, implements and directs the Case Management Department.
Essential Responsibilities:

- Directs and supervises case management, social services, and revenue integrity personnel.
- Promotes quality, cost effective care through management of access-to-care in the Emergency Department, progression of care, barriers to delivery of care, and transition to lower level of care or discharge back to the community.
- Utilizes extensive and creative planning and resource identification to enhance discharge planning for patients with limited financial and supportive resources.
- Directs the implementation of utilization management and regulatory compliance.
- Promotes evidence based care through physician partnership.
- Advocates for patient/family to meet their social and emotional needs as they impinge on their medical condition, treatment, recovery, and safe transition from one care environment to another.
- Provides crucial services to children who are suspected victims of child abuse through the Child Protection Team.
- Educates the health care team on resources, regulatory compliance, and financial issues.
- Demonstrates quality and financial outcomes for the department.
General Responsibilities:

Minimum Qualifications:
Education: Bachelor’s Degree in applicable field required.
- Nursing Candidates: Bachelor of Science in Nursing (BSN) from an accredited school of nursing required with an education plan to obtain a Master of Science in Nursing (MSN) within five years.
- Social Worker Candidates: Master of Social Work (MSW) required.
Experience: 5 to 7 years of progressive leadership experience required with at least 4 years of professional work experience in case management.
License(s)/Certification(s)/Registration(s):
- Nursing Candidates: Current RN License issued by the Oklahoma State Board of Nursing, or a current multistate compact RN License (eNLC) required.
- Social Worker Candidates: Current Licensed Clinical Social Worker (LCSW) from the Oklahoma State Board of Licensed Social Workers required.
Knowledge, Skills and Abilities:
- Knowledge of nursing practices and procedures.
- Knowledge of the principles and practices of social and psychological casework.
- Must be knowledgeable about resources available in the community for use by patients, families and the health care team that would facilitate in the recovery of the patient.
- Requires knowledge of third party payer issues concerning reimbursement and regulatory perimeters.
- Must be able to explain technical and complex information in such a way that patients and family can understand.
- Strong interpersonal skills that facilitate the gathering of information necessary for assessing the patient’s needs and the range of treatment and care necessary.
- Strong clinical assessment skills.
- Analytical skills sufficiently strong to interpret situations and determine the appropriate course of action.
