Job Description

About The Role: 

The Nurse Care Coordinator will be responsible for coordinating initial nursing care for our infusion patients and monitoring the ongoing home health nurse/patient relationship and patient progress. The position will include establishing and maintaining positive relationships with patients, home health agencies and referral sources. You’ll work collaboratively with sales, intake, pharmacy, and outside agencies to ensure our new patients receive the level of care that KabaFusion is known for.

Essential Duties & Responsibilities: 

• Review and approve nursing notes and invoices submitted by contracted nursing agencies.
• Evaluate and intervene in nursing-related patient care issues

Qualifications

What you bring:

• Current and unrestricted LPN license in the state of employment
• 3+ years of experience as registered nurse in a home health agency
