SENIOR COMMUNITY HEALTH NURSE

Your Specific Responsibilities:

 

This position performs professional Public Health Nursing work in a specialized area requiring training beyond that required as a Registered Nurse. This position performs professional nursing duties and responsibilities in the School Health Program in Hernando County School District and works independently.

 

Ensures that no student will be admitted or attend any public or non-public school without proper documentation; performs record reviews to determine compliance with the following:

1. Certification of Immunization or Certificate of Immunization Exemption.
2. Health examination within the past twelve months for all students entering Florida schools for the first time.

 

Manages health care issues of students concerning actual or potential health problems which include but are not limited to:

1. Nursing Assessments and triage including rapid assessment of acute physical problems when necessary.
2. First aid and emergency treatment.
3. Vision, hearing, scoliosis, and growth and development screenings.
4. Obesity and nutritional counseling and education.

 

Ensures that a Cumulative School Health Record is established for each student and the record includes documentation of identified health problems and corrective measures taken.  Completes a separate professional treatment record to be used to document the provision of confidential health services by the health care professionals.

 

Assists in identification of high-risk students with medical issues requiring assistance and possible emergency intervention in the school setting.  Individual Care Plans and Emergency Care Plans will be initiated and shared with appropriate staff.

 

Acts as liaison in communication of health-related problems (within the limits of client confidentiality) between parents, teachers, coaches, other health providers and students (including home visits as needed) to form a collaborative effort to meet the needs of the child.  When working with middle and or high school youth, provides a teen-friendly approach to student’s medical and emotional needs while advising in a professional, organized and confidential manner.

 

Provides health education to families, school personnel, students and refers for services when appropriate.  Provides age-appropriate nursing counseling as needed.

 

Acts as a resource person/nursing consultant (within the limits of DOH-Hernando policies) and provides health information for school health professional, teachers and other school staff.  Participates as a member of the School Health Advisory Board.  Collaborates with school and community support services in order to facilitate the oversight of student health and improve health outcomes of students.

 

Assists in developing a procedure for administration of medications during school hours. Ensure that medical services (invasive procedures) are provided by appropriately trained individuals and these individuals are evaluated annually for competency. Provides Medication Administration classes to School Health Professional as well as all staff designated by the principal to administer medications annually. Additionally, provides training for “Field Trip” medication administration annually. Provides education as needed to students permitted by physician and parent to self-administer medications.   Ensures that physicians’ orders are in place; takes and implements physicians’ orders.
