Medical Assistant in New Primary Care Model for Dialysis Patients

DaVita is pioneering a new approach to primary care for patients with end-stage renal disease (ESKD) directly within our dialysis centers. We're seeking a highly motivated Medical Assistant to help us expand this groundbreaking program.

It is estimated that 30-50% of patients with renal failure do not receive consistent primary care support, resulting in increased morbidity, hospitalizations, and mortality. You will work with a Nurse Practitioner to address the health barriers patients with kidney disease face. Our goal is simple: keep patients out of the hospital and living longer. Come join us!

  • Position yourself to be a national leader: We plan on scaling this program nationally and are looking for individuals who will grow with the model.

  • Have an outsized voice in shaping the model of care: We will be nimble and agile in building a model that meets the needs of our patients and are looking for our MA to add his or her thoughts to how we can improve.

  • A true partnership: Work in a close, collaborative relationship with a dedicated Nurse Practitioner.

  • Competitive pay and benefits packages: We strive to do inspiring work with financial and personal balance.

  • Complete non-clinical screenings and pathways under the guidance of the Manager; escalate to Integrated Care Nurse as needed

  • Assist the clinical team with moving patients through the continuum of care by completing tasks as directed

  • Manage clinical data so that key clinical reports can be produced. This includes transferring information from outside medical records and auditing data for clinical integrity.
