Pediatric Home Service is launching our new Hospital to Home program, and we’re seeking a dedicated Medical Social Worker to help medically complex children safely transition from the hospital back to the place they belong—home.

THE POSITION

• Location: Roseville, MN
• Hours: Part-Time
• Requirements: LMSW license in good standing, ability to travel to facilities and conduct in-home visits, 3–5 years of experience in a healthcare-related field

Why You’ll Love This Role:
As part of our Hospital to Home team, you’ll provide vital social work support to ensure timely hospital discharge, prevent rehospitalization, reduce emergency department visits, and promote school attendance. You’ll work closely with our clients, families, and our extended care team to ensure children thrive in their home environment.

ESSENTIAL FUNCTIONS

• Collaborate with physicians and healthcare teams to understand social and emotional factors affecting patients’ health.

• Guide patients and families through individual or group sessions to understand and follow medical recommendations.

• Develop and implement service plans aimed at restoring patients to optimal social and health adjustment.

• Connect families to community resources to support life at home or within disability limits.

• Prepare patient histories, service plans, and reports.

• Participate in program planning to improve health services by interpreting social factors.

• Complete required documentation and paperwork accurately and on time.

EXPERIENCE & CERTIFICATIONS

• Licensed Social Worker with 3-5 years of experience in pediatric or medical social work.

• Strong communication and collaboration skills to work with families and interdisciplinary teams.

• Passion for supporting medically complex children and their families during critical transitions.

SALARY RANGE

At PHS, we are committed to providing specialized care for medically complex children. Every role, from direct patient care to customer support, plays a vital part in improving the lives of kids and families. The impact you make is invaluable. We are dedicated to offering fair and transparent compensation to our team members. The range for this role is $27.16 to 32.60, based on experience and qualifications.
