HSNT is seeking a Medical Care Manager that is passionate providing ongoing support and effective communications, and accountability within the Care Management team. The Care Management team provides education, and resources in a community health setting to patients and families that may have very limited options for their medical needs.


Our Medical Care Manager becomes a part of a tight-knit, interdisciplinary medical team, which removes biopsychosocial barriers and supports patients. The Medical Care Manager’s primary responsibilities are coordination and monitoring of Ryan White program and services, coordinating client health care for persons with HIV /AIDS throughout the continuum of care, assisting Ryan White Program manager in execution of duties, and providing medical care management services to Ryan White patients.

Individuals that are successful in the Medical Care Manager role tend to be an adept leader, customer-focused and detail oriented, possess a true desire to serve a diverse population of patients and can do so without bias, and demonstrate excellent communication skillsets. It is also helpful if candidates have knowledge of some community resources or programs and have some familiarity with various databases and EMR systems, along with Microsoft Word and Excel. 


A day in the life of our Medical Care Manager may look like this:


  • Coordinate client health care for persons with HIV or AIDS through the continuum of care; monitor and advocate for access to referrals and health care services. Will maintain a case load of active client’s appropriate units of service as determined by HSNT.
  • Provide day to day oversight of programs and support care management team across all HSNT medical centers. Resolve issues encountered during daily operations and determines appropriate solutions. Provide guidance and expertise to program staff in determinations of difficult and/or complex cases and mediate, if necessary.
  • Assess program related initiatives, evaluate, and identify opportunities and weaknesses within programs and address appropriately. Assist with implementation strategies for workflow optimization.
  • Monitor client metrics at the direction of the Program Manager and/or Lead Manager.
  • Responsible for billing and reporting elements and monitoring client metrics: end of month billing, monthly/quarterly reports/performance measure reporting/audit management/Peer Review reporting, call queues, etc.
  • Responsible for organizing staff development, education, and training.
  • Assist with grant writing and related grant compliance.
  • Assist with selection, development, assessment, and retaining care management staff.
  • Participate in Quality Management meetings/studies as requested.
  • Ensure adherence to standard and departmental program policies and procedures, established standards of care and continuum of care to accommodate care management goals.
  • Perform other tasks as assigned by the Director of Programs or Program Manager. 

Requirements

  

We ask that our Medical Care Managers have the following:


  • Education: Bachelor’s degree in social work or related field.
  • Minimum of (4) four years of progressive responsibility in utilization review/care management or social work in a healthcare setting.
  • Experience in HIV education or Care Management preferred.
  • Must possess a valid driver’s license.
  • Ability to create and maintain relationships with community service programs.
  • Strong communication skills both verbal and written.
  • Ability to relate to individuals from a variety of racial, ethnic, religious, and cultural/socioeconomic backgrounds, including various sexual orientations.
